A multidisciplinary team that provides for the needs of mentally ill seniors 65 and over. Senior Outreach is comprised of two teams, one located in Ventura and one in Thousand Oaks. The goals of Senior Outreach are to make mental health services accessible to seniors, to prevent inappropriate institutionalization, and to maximize independence. Senior Outreach has several components including assessments, crisis counseling, medication evaluation and monitoring, psychological testing, individual and group therapy, and case management.
